I think it's fake too... just the way the boob comes rounded down to her shirt.. and suddenly seems to indent right at the nipple.
If you follow the path of the boob from the top down, you see, just above the nipple... the shirt suddenly has a black line that would go exactly where the boob would go if it was still following the same path, but instead, the boob then curves inward.
I have to admit though, it's a real good faking job.
